CS Undergraduate students who intend to be Readmitted from AWOL, Readmitted from Dismissal from the Degree Program, Readmitted from Dismissed status (Double Probation/24 unit rule violation) / Zero Passing and Readmitted for Non Major status, SHOULD FILL OUT THIS FORM FIRST AND SUBMIT THE HARD COPIES OF REQUIREMENTS PREFERABLY TWO (2) WEEKS BEFORE THE ONLINE PRE-ENLISMENT (to avoid delays). 

DEADLINE FOR SUBMISSION OF ALL DOCUMENTS (HARD COPIES): 21 JULY 2026

(This process will take at least a week so returning students must start the process as soon as possible)
