Parent, Teacher, & Student Conference Request
Conferences take place on Tuesdays and Thursdays from 8:50am to 9:15am.

Please complete the following form so we can schedule a Parent/Teacher conference as soon as possible.
Parent/Teacher conferences can be face-to-face or virtual via WebEx at the request of the parent/guardian. If WebEx is chosen, a link will be emailed to the email address included in this form.  We encourage students to be a part of the conference. 

*If you are requesting a meeting to discuss ESE or 504 accommodations, DO NOT FILL THIS FORM OUT. Please e-mail  your counselor directly.

We have found that it is extremely helpful to have the student attend these conferences. Please indicate whether your student will attend the conference below. *
Parent/Teacher/Student Conferences are on Tuesdays & Thursdays from 8:50 AM and need to conclude by 9:15 AM. When scheduled for a conference, please arrive NO LATER than 8:45 so that you can sign in and get to the conference location on time. *
We conduct conferences on Tuesday and Thursday Mornings at 8:50 AM. If for have a unique circumstance that prevents scheduling at this time, please note it here and we will try and accommodate you.  We cannot conduct parent/teacher conferences during school hours.
