Online Survey Consent Form

You are invited to complete this voluntary online survey. This survey is being conducted by two educators, Megan Storey Hallam and Daniel Cruz.
 
The purpose of this survey is to gather answers to many of the questions educators are asking each other during the COVID-19 pandemic. Our aim is to share these results with you as a community of educators. A secondary purpose is to provide direct, firsthand information to administrators, researchers, and policy makers regarding the various experiences of educators across the country and around the world.
 
In order to provide the most accurate results, we ask that only current educators complete this survey and that each educator complete it only one time. We will not gather any personally identifiable information and the survey should take about 15 minutes to complete.
 
Although you may not directly benefit from participating in this survey, we anticipate that you will find the results informative in your practice as an educator during this situation. We hope that your participation will provide helpful information to educational leaders and policy makers as they develop and recommend educational plans during this crisis.
 
We are not collecting any personally identifiable information and we believe that there are no known physical, psychological, or social risks associated with your participation. We will take all appropriate steps to ensure that your answers will remain confidential.  We will minimize any risks by reporting only aggregate data and not associating any results with identifiable information.
 
You are under no obligation to complete this survey and can stop answering questions at any point.
