Alternate Transportation Request Form: Approved alternates remain in place until the end of the school year, or until a replacement request is submitted and approved.
Please complete this form if you are asking for your child to be picked up or dropped off at any address other than your place of residence, OR, if your child does not require bus service.

ALTERNATE TRANSPORTATION POLICY
If you are requesting that your child be picked up or dropped off at bus stop assigned by using any address other than your residence, you must submit a request for alternate transportation form to the transportation office.

Completed forms must be received in the Transportation Office by July 15 to guarantee beginning the first day of a new school year.

After July 15, and for the duration of a school year, forms must be turned in at least three (3) school days prior to the requested beginning date. All requests submitted after July 15 are subject to bus & route availability.
