The Family Education Rights and Privacy Act (FERPA) affords certain rights to students concerning the privacy of, and access to, their education records. Students may choose to complete and submit this form to allow the release of their education records to specified third parties. Please note that while this form authorizes your institution to release education records to third parties, it does not obligate your institution to do so. Each institution reserves the right to review and respond to requests for release of education records on a case-by-case basis. Each Carmen student agrees to stay connected with our Post-Transition program and completes a FERPA waiver. If students fail to self-report grades by a scheduled date, they understand and agree to let Carmen, using their signed FERPA waiver, request their grades directly from the college or university.
