Report Form for Sexual Assault, Sexual Harassment, and Sexual Misconduct
Thank you for reporting. Your safety is of the utmost importance to us. If you wish to make a formal Public Safety report or police report, OR if this is an EMERGENCY, immediately contact the St. Catherine University Department of Public Safety at 651.690.8888 or your local police department by dialing 911. Contacting police does not require you to pursue legal action.
The Title IX Coordinator and Deputy Coordinators are notified when reports are made via this form. Typically the details of the form submission with be reviewed no later than the next business day.
This form is intended for use by any member of the St. Catherine University community to report specific information related to incident(s) of sexual assault, sexual harassment, and other sexual misconduct.
This is a secure report accessible only to the appropriate St. Catherine University officials, including the Title IX Coordinator and Deputy Title IX Coordinators. You are not required to complete the entire form in order for the report to be submitted. Your report will trigger an investigation. However, the University's ability to respond effectively may be limited by the level of information provided and/or the anonymity of the complainant. All reports will be referred to the Title IX Coordinator and the Deputy Title IX Coordinators.
For the victim/survivor completing this form, it is your choice whether to remain anonymous. However, please know that doing so may limit the University's ability to address and assist you. We strongly encourage you to access available University resources, such as the Student Affairs office (651.690.6778), the Counseling Center (651.690.6805), and the Health and Wellness Clinic (651.690.6714), or off-campus resources such as SOS Sexual Violence Services (651.266.1000) or the Sexual Violence Center (612.871.5111).
