We ask that you only use this form when you have a request that meets or exceeds $750, as this is intended for large orders of specific items that you need to secure in large quantities more than a week before your event.
After this form is completed, you will be notified by CTFC staff. If you have multiple different events with different desired pick-up dates, you will need to complete a separate form for each.
Please note, after your order has been placed on Thursdays, you can only request to remove items. Any items you wish to add must be ordered separately via regular preorder protocol.
