Thank you for reaching out to the Transgender Legal Defense & Education Fund (TLDEF)'s Name Change Project. We assist in connecting low-income TGNCNB (transgender, gender non-conforming, and/or non-binary) people with law firms that provide them with pro bono (free) legal representation for name changes in the cities listed below.
If you are a low-income TGNCNB person seeking to get your name legally changed and live in one of the participating cities below, please fill out this screening form.
The information you submit via this form will be considered by TLDEF in matching you with a cooperating law firm. It will only be shared with cooperating law firms who we have determined may be able to assist you in changing your name. Except for providing your information to cooperating law firms, TLDEF will keep the information you share with us confidential.
In addition to meeting income-eligibility guidelines, project participants MUST LIVE IN one if the following cities or counties to participate in the project:

YOU MAY ONLY FILL OUT THIS FORM YOURSELF OR ON BEHALF OF A MINOR CHILD OR PATIENT
TLDEF can only accept requests for legal representation directly from the person seeking legal assistance. However, if you are the parent, guardian or "next friend" of a TGNCNB youth or the case worker of a TGNCNB patient, you may fill out this form on their behalf.
WHAT HAPPENS AFTER I SUBMIT THIS FORM?
Once you submit this form, TLDEF will review it and advise you whether we are able to match you with a cooperating volunteer lawyer. At that time, we will request other information required for your representation. TLDEF tries to respond to all legal assistance requests on a first come, first serve basis.
It will take up to 15 - 20 business days for you to receive a response once you submit the form. This response will arrive either by phone, e-mail, or text message. Once you have received a response and an intake has been completed, there will be additional waiting time prior to the beginning of representation.
NO LEGAL REPRESENTATION BY TLDEF
Filling out this form does not establish a lawyer-client relationship with TLDEF. TLDEF does not take on name change clients directly, but operates the Name Change Project in partnership with private law firms and corporate in-house counsel in the above-listed cities and counties. If TLDEF determines that you are eligible for assistance, you will be represented by your assigned lawyer for your name change only. If you do not live in one of these jurisdictions, the lawyers we work with will be unable to assist you with your name change.
Note: The Name Change Project cannot provide referrals to individuals with unresolved warrants. If you have open warrants, they must be cleared before you can be determined eligible for participation.
