This form is for students who need NEW bus service OR students with a NEW address different from last year. 

If your child has previously ridden the bus and NO changes have been made, then it is NOT necessary to fill out this form. If you have any questions, please call the Transportation Department at 254-694-4121.

Please Note:
If you have a change of address, please contact the Transportation Department so your student can be reassigned to the correct bus. 

They will be denied boarding until they have a bus pass issued by our office. 

Make sure all of your contact information is kept updated. 

All Elementary and Special Ed students MUST have an adult present at their bus stop, otherwise they will be brought back to the bus barn and it will be your responsibility to pick them up there.
