After submitting this registration, send a check for $3,675 (reflects $1,000 deduction from recent grant) or $2,985 (reflects $1,000 deduction from recent grant) for the Early Bird Discount if registration received before June 1st, 2022. You may also pay by credit card upon request.

Course fee covers the 25 CME course, lodging, all meals, and water activities.

Cancellation Policy: Full refund is granted if cancelation is received 60 days before event. We understand that life challenges emerge and changes happen. In the event that an emergency arises and a cancellation is needed after the 60 day window, we will do our best to fill in the reservation with another physician. We will be glad to refund the entire amount if the registration is replaced.

COVID-19 Vaccination Requirement

To protect participants and support staff against infection with SARS-CoV-2, verification of full vaccination status will be required according to the current CDC recommendations at the time of the in-person gathering.  Acceptable proof of SARS-CoV-2 vaccination includes a CDC vaccination card, a photocopy of the card or a photo of the card on a mobile device.
