DOE & Charter Staff IEP Translation Request Form
In September 2018, the Department of Education’s Translation & Interpretation (T&I) Unit launched a program that now provides all CSE staff, administrative district 75, and schools with central support to translate Individualized Education Programs (IEPs) for the current school year at no cost to families.

The T&I Unit will process these requests after the IEP is finalized in SESIS.

To REQUEST the TRANSLATION of an IEP - Click the NEXT button at bottom left of the screen.

If a parent requests a translation of any other special education-related document (such as PADs, RADs, PWN, assessments), the school must continue to provide translation as required by Chancellor’s Regulation A-663. Schools and CSEs can do so with DOEs contract vendor, Lingualinx, (Contract Number: 9078350 Vendor Number: 010749117) by completing one of the following translations request forms:

For 504 Plans, 504 Coordinator may submit via the Online 504 Tracking Form https://docs.google.com/forms/d/e/1FAIpQLSfr2y7xc1ISp9nrC4kzIkFcAtI2pHyp3HpNtL_jxGJ4-mxBSA/viewform.
