Please complete this permission form to give your student(s) permission to leave the high school campus during the school day for the specified events/times listed.  

Potential reasons that students may need permission to leave campus during the school day include A+, Assemblies, Broadcasting, Career Center, Classes at LHS, Early College Program, Early Release Day lunches, Educational Internship Program, FLEX time, Liberty Virtual Study Hall, Marketing Internship, Network 53 Internship, Newspaper, Practical Leadership, ProStart, Sports Marketing, or Yearbook.

NOTE:  We do not have an open campus for lunch.
