Complete this form and pay total online here (Under Fund, scroll down and choose "Christmas Wreaths"). Or you can make a check  payable to Church of the Servant, “Christmas Wreath” on the memo line and mail to Church of the Servant, 4925 Oriole Drive, Wilmington NC 28403. 

Orders must be received in the office by November 19th. 

Wreath(s) will be available for pickup at the Church on:
Friday, December 5th - 10am to 2pm
Saturday, December 6th - 2pm to 4pm
Sunday, December 7th between services and after 11:15 service 

Delivery Guidelines: Although we cannot offer general delivery, we will deliver to shut-ins and others who are unable to pick up their orders due to an emergency or illness. Please include:
1) Name of person receiving the wreath
2) Address of delivery
3) Phone or other contact information to confirm delivery
