BUS STOP CHANGE REQUEST FORM

In order to capture accurate and detailed information, we are asking parents/guardians to complete the form below for any bus stop change requests throughout the course of the school year.

Forms will be reviewed by the Transportation Department in the order they are received. A decision will be communicated within (30) days, based on submission date. A Bus Stop Change that is granted will be effective-dated and will NOT be in place for the next day of school.

For bus stop changes for child care purposes, please refer to our Childcare Transportation Request Form, located on the Transportation Department page of the Nazareth Area School District website.
