COLLECTION OF VACCINATION INFORMATION
Consistent with its duties under the Occupational Health and Safety Act 2004 (Vic), the Bicycle Recycle Shed Incorporated (ABN 65 947 482 012, also known as the Bike Shed at CERES) requires volunteers to be fully vaccinated (or exempt) prior to attending the Bike Shed.

Your vaccination information will be stored in accordance with the Health Records Act 2004 (Vic), and only used for the purpose of complying with the Bike Shed's duties under the Occupational Health and Safety Act 2004 (Vic).

Please contact the Bike Shed by email to manager@thebikeshed.org.au if you have any queries, or are exempt from the requirement to provide a vaccination certificate.

If you do not complete this form (and do not have a valid exemption which has been formally accepted by the Bike Shed), you will not be able to attend the Bike Shed.
