MEMBERSHIP AGREEMENT AND LIABILITY RELEASE
I agree:

1.  to be fully responsible for the payment of all charges incurred for membership fees, meet fees, and other charges to my account.  I understand that Toronto School of Golf reserves the right to terminate my membership where unpaid account balances remain outstanding for over thirty (30) days and that  Toronto School of Golf may turn over delinquent accounts to a third party for collection

2.  that if I terminate my membership or my membership is terminated by  Toronto School of Golf during the training season, I will be required to pay an additional $50.00 before I/my child(ren) is(are) permitted to recommence training.  I agree to notify Toronto School of Golf  Coaches immediately, in writing (email), if I find it necessary to cancel my membership.  I understand that I will be responsible for the ongoing fees until I give notice.

3.  that  Toronto School of Golf, its officers, directors, coaches, employees and volunteers, the City of Toronto Parks and Recreation Department, and the Toronto District School Board shall not be liable to me, my child(ren), my/his heirs or estate, for any claims, demands, injuries, damages, actions or causes of action whatsoever arising, whether through negligence or otherwise, and whether with respect to personal injury, destruction, theft to or of property or otherwise, sustained by me/my child while attending or travelling to or from any practices or golf tournaments or while being at a summer/winter camp or participating in any other Toronto School of Golf activity.

4.  that in signing this waiver, that I voluntarily assume all risks associated with me/my child's participation in  Toronto School of Golf and that Toronto School of Golf will not be held liable for any claims, demands, injuries, damages, actions or causes of action whatsoever incurred by any members of my family or persons attending  Toronto School of Golf facilities or activities on my behalf.

CODE OF CONDUCT

  • All Registrants are responsible for maintaining and enhancing the dignity and self-esteem of Registrants and other individuals based on the principles of the Positive Coaching Alliance:
  • Demonstrating respect to individuals regardless of body type, physical characteristics, athletic ability, age, ancestry, colour, race, citizenship, ethnic origin, place of origin, creed, disability, family status, marital status, socioeconomic status, gender identity, gender expression, sex, and sexual orientation
  • Focusing comments or criticism appropriately and avoiding public criticism of athletes, coaches, officials, organizers, volunteers, employees, or Registrants
  • Consistently demonstrating a spirit of sportsmanship, sport leadership, and ethical conduct
  • Acting, when appropriate, to correct or prevent unjustly discriminatory practices
  • Consistently treating individuals fairly and reasonably
  • Behaving in a respectful way to the coaches and other participants
  • Not damaging, breaking, or stealing the property of TSG
  • Three strike rules will be applied for misbehavior, disrespect, and/or violence towards the coaches, athletes, parents/guardians.
  • We have the right to terminate your membership if these rules are violated

I understand that photographs and videos may be taken of the golfers from time to time during Toronto School of Golf activities. I agree that Toronto School of Golf may use such photographs and videos for promoting Toronto School of Golf

In accordance with Canada's Anti-spam Legislation, I give express consent to Toronto School of Golf to use the email addresses provided on this registration form to communicate club related information.

 Toronto School of Golf Program Fee Structure
  • 1 hour, once per week -  $140 a month (for 5 weeks month $175)
  • 1 hour, twice per week - $240 a month (for 5 weeks month $270)
  • 1 hour, three per week - $330 a month (for 5 weeks month $365)
  • Private lessons - $65/hour
 Make-Up Classes
  • Make-up classes are only offered when written or verbal notice provided 24 hours before your class
  • Are not guaranteed & based on availability
  • Cannot be carried over more then 2 weeks
  • Have no cash value
  • Up to 1 make-up class per child per month
