Absence Forms
In the spirit of professionalism and preparedness, all absences must be approved 30 days prior to your anticipated absence. Along with submitting an absence, all staff members must create a work plan and find their own substitute. The work plan includes thorough instructions for the designated substitute to be successful in your role. Absences are not paid, instead, the absent staff member will not be required to perform any of their responsibilities for the entire week. Failure to have an approved absence and work plan will result in termination.
