This questionnaire is designed to help us understand what measures are needed for our Faith Family to return to celebrating the Eucharist in person at Our Lady of Guadalupe Church. We would like the head of household to complete this survey; but, ALL other members of the household (16 years old and older) may complete an individual survey on the website or in church.

Note: Current COVID Regulation: The City of New Orleans has recently moved to a modified Phase III, which allows Houses of Worship to operate at 75% capacity. For OLG, this means our church could have as many as 225 worshippers. In order to increase seating seating within the regulations set by the City, we will move forward with reducing the number of pews that are completely blocked off. Which would you prefer?

That the church were divided in half with one-half of the church maintaining 6-ft. social distancing (every other pew would be blocked off) and the other half would follow civil capacity requirements (fewer than 6 ft. distancing, with no pews blocked off)

That the front 14 pews on both sides would maintain social distancing with every other pew blocked off, and the back 6 pews on both sides, following civil capacity requirements (fewer than 6 ft. distancing, and no pews blocked off).

NOTE that we will monitor capacity and not exceed the 75% capacity limit regulation, and we will have the opportunity for our members to go into the St. Jude Hall, where Mass is live-streamed and the Eucharist is distributed. We look forward to the day when our entire Faith Family can celebrate the Eucharist together in person.
