47th ward mask requests
Beginning May 1st, Illinois residents are required to wear masks or coverings when in public. If you need a mask, please fill out this interest form. We are working to donate masks to those in-need. While we will do our best to match you with a mask, we cannot guarantee that we will be able to provide one. If you are able to afford purchasing a mask from an artist or local business owner, we recommend checking with your local Chamber of Commerce for recommendations.
We will prioritize requests from seniors, high risk individuals, and essential workers. Please note that if you live in a senior living community we will be delivering masks directly to your facility. There is no need to fill out this form.
AS A REMINDER, WE HAVE A LIMITED NUMBER OF MASKS. PLEASE ONLY SUBMIT THIS FORM IF YOUR HOUSEHOLD IS IN NEED, so as to not risk leaving someone who cannot provide their own without one. Some categories that may qualify as in need:
- 60 years or older
- Serious underlying health conditions (like diabetes) that put you at high risk for COVID-19
- Essential workers who must report to work IN PERSON
- Low income or waiting to receive unemployment benefits
- Currently treating a chronic illness like cancer
- Limited mobility or difficulty traveling
Because we have a limited quantity of masks, we cannot fulfill requests for those who live outside the 47th ward. We are referring requests from those who live outside ward directly to their Alderman. If you have any questions, please feel free to reach out at
