2019 Transportation Policies

All forms must be submitted 24 hours prior to the travel date(8:00am Prior to the date of service). All rides must cancelled two hours prior to the pick up time or the rider agrees to pay for the ride as if it was completed. No one other than the riders on the trip manifest will be allowed to ride. All riders must show their ID's and they must match the Full name of the booked rider(School ID's will be accepted). There can be no change in the route when the trip has started.

All Transportation Requests must be submitted using the “Transportation Request Form” provided here. If you need to submit via email, Please copy, paste, and complete the form as the body of your email. Information provided in the request must include the following: Requester name and contact information, date of travel, number of passengers, a passenger contact name with cell number, times and places of departure, destination.

Incomplete requests may not be considered, and no transportation will be confirmed until all information is provided.

Requesters will receive an email Confirmation of the trip when it is approved and scheduled. Trips shall not be booked until the Driver has received and carefully examined the Transportation Confirmation.

Only a limited number of off-route deviations can be made on any given trip. Vans can deviate from their fixed routes to serve other locations within the service area, but they cannot necessarily provide door-to-door service due to safety and other operational considerations (narrow streets, cul-de-sacs, etc.). You may be requested to board/deboard at a location a block or more from your home or destination.

Estimated pick-up and drop-off times will be provided at the time you book. Please be waiting at the curbside 5 minutes before the given pick-up time.

I hereby release, waive, discharge and covenant not to sue the T.A.P.S, Inc.(Teaching Athletes Plans for Success), the Board and its individual members, officers, agents, servants, or employees (hereinafter referred to as releases) from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my minor child(ren) or me, or any of the property belonging to me, as result of, or in any way arising out of my child(ren) traveling to and or from any destination and travel by T.A.P.S Transportation via any van, car, bus and mode of transportation.
2. I voluntarily assume full responsibility for any risks of loss.
3. I further hereby agree to indemnify and hold harmless the releases from any loss, liability, damage or costs due to my child(ren) traveling to and or destination and travel by T.A.P.S Transportation via any van, car, bus and mode of transportation, in a vehicle or vehicles not owned by T.A.P.S, Inc.
4. I hereby further agree that this Waiver of Liability and Hold Harmless Agreement shall be construed and enforced in accordance with the laws of the state of NC, SC, GA, FL.
5. In signing this release, I acknowledge and represent that I have read the foregoing Waiver of Liability and Hold Harmless Agreement, understand it and sign it voluntarily as my own free act and deed
