This evaluation checklist should be completed by the appropriate Department Chair, Program Manager, Lead content faculty member or designee. Ratings should be assigned using any combination of the following items: Student evaluations, completion rates, communications with faculty, students and staff, direct observations, review of course materials, syllabus, Canvas site, student assessments, etc.
