For ages 8-14

Please complete this quick form to request registration for your child.  A more comprehensive form including emergency contacts and medical information will be emailed to you and must be returned before the first day of class.  

The cost of this six-week course is $130.  $30 is due at the time of registration and can be sent via PayPal or Venmo.  The balance must be paid in full before the first class, unless you have made other arrangements with me.  (If you are experiencing financial hardship, I am happy to work with you on a payment plan or reduced fee.)  

You will receive an email confirming that your registration request has been approved along with an invoice.
