Please fill out the form in order to donate your device. Once we get your information, we'll email you a UPS label so you can ship the device to us—on our dime. (Please note that we can only provide labels for shipment from US mailing addresses.)

By filling out the form, you agree to the following:

* Due to the nature of creating repair guides, we cannot send your device back to you.

* You will not hold us responsible for wiping all your personal data from the device. We'll do our best to wipe your information, but ultimately that responsibility falls onto you.

Our pledge: all of the information below is only going to be used to set up the UPS label, and will not be sold on the black market.
