Thank you for your interest in our discounted sterilization services! This form will put you on our contact list for future clinics in your area, including with other clinics such as SLV Spay and Neuter Alliance, Agents of Chaos or Pueblo Spay and Neuter Now, and it will also act as a waiting list for last minute appointments. Make sure your information is 100% correct as we will email, call, or text you when clinics are being scheduled or when we have last minute openings. *Our notification emails do sometimes go to SPAM, so keep an eye out!

We will keep you on our list for one year from your signup.

* AGE: pets must be between 8 weeks and 7 years old to be considered for surgery.
* WEIGHT: pets must weigh at least 2lbs at the time of their appointment.
* HEALTH: We do not offer appointments for animals not in generally good health, those with a history of neurological complications and/or some other health issues. Some conditions may put your pet at a higher risk of an adverse anesthesia event or other complication. You must disclose these issues for the safety of your pet.

* IN HEAT: Pets that are nursing offspring or in a heat cycle, can usually still receive spay/neuter services. Please contact us at 719-657-7115 if you have concerns or questions.

* REQUIRED SERVICES (given at our clinic at no additional charge, if needed):
~ FVRCP (CAT) vaccine ~ *Required for all cats that are not current on this vaccine.
~ DHPP (DOG) vaccine ~ *Required for all dogs that are not current on this vaccine.

~ RABIES (CAT & DOG) ~ *Required for all pets aged 3mos + not current on this vaccine.

~ MICROCHIPS (CAT & DOG) ~ *Required for all pets that are not chipped currently

(Feral/Barn cats receive an ear tip instead of a microchip).

*WE DO NOT OFFER DISCOUNTED SERVICES TO PETS NOT PERSONALLY OWNED, such as RESCUE GROUPS, ORGANIZATIONS or BREEDERS. If you fall into one of these categories, please send an email request to: signup4bergen@gmail.com and one of our clinic volunteers will reach out to you about pricing/availability.
