Alumni should use this form to request a high school transcript to be sent directly to a school, employer or other organization.
Current students should visit the guidance office to complete a transcript request form document in person.
Please note. All transcripts must be sent directly to an institution or organization to be considered official. We are not able to email transcripts to personal email addresses. If you need a copy of your transcript provided to you in person or via mail, you must complete a signed release form available in the guidance office. You can also request a copy of the release form via email by sending an email to Mrs. Jacqueline DeSantis - jacqueline.desantis@gcsny.org
