I'd Like to Retake a Test with Mrs. McDevitt
You are welcome to retake any math test this year.

This retake process isn't designed to be easy to complete.  It asks you to give careful consideration to your preparation for the first exam, analysis of each test problem, set a goal and to create an action plan to help you achieve your goal.

I want you to do your absolute best on each test and retake - this usually requires a significant amount of preparation.  If you don't put in the time and effort you probably won't do much better on the test retake.

Please meet all deadlines.
This online RETAKE REQUEST must be submitted within 1 day of taking the retest.
You will have one week to turn in your ERROR ANALYSIS once you have received your initial test back.
The RETAKE EXAM must be taken at lunch on an teacher assigned date approximately one week after the initial test has been returned and gone over thoroughly in class.
Your homework packet and pointsheet must be turned in with all unfinished homework completed.  You are not eligible to retake the test if homework has not been fully completed and turned in prior to the retake.

Please ask if you have any questions.

Error Analysis
Please correct each problem you missed on the test on a separate piece of paper even if you only missed a small portion of it.

All work must be shown and done properly.

Please ask for help if you need it.  We can do them together!

Please explain why you missed the question ("I added when I should have subtracted.").

**Ask a parent to sign your original test.**  They want to help you do your best and this way I'll know they know you have a plan!

Staple the Error Analysis to the back of the original test.

**Turn your Error Analysis in within 2 schools day of submitting your RETAKE REQUEST.**  Please staple it to the original test and turn it to Mrs. McDevitt directly - do not put it in your 'turn it in  basket,' her desk or her box in the office.  Thanks!
Action Plan
If you'd like to improve you have to practice.  Ask any successful musician or athlete - they all practice!  Mastering a skill takes practice and lots of it!

Do a little bit every single day and it won't be as bad as doing 1 - 2 hours (or more) in one night!
I understand the retake test is a different and slightly more difficult exam. *
I understand I will keep the higher grade of the two tests. *
I understand I will need to turn in all of my practice (homework and study guide) in order to receive the retake exam. *
I plan to do the following in order to prepare for the retake exam. *
You must indicate at least one but may choose as many as you'd like.  Let me know if you'd like help working through the problems, finding websites or practice worksheets, etc.
Required
Retake Date
Retakes must be completed before the end of that specific trimester that math unit was taught and tested.

Retakes are usually taken during lunch approximately one week after the original test was handed back. *
Do you agree to take it during lunch?
If you answered "no", please pre-arrange with me for another time after school.
