Sample ESIC Intern Evaluation Form
The goal of the Environmental Sustainability Internship Course (ESIC) is to help students develop leadership and project management skills  in the context of improving environmental sustainability in our community.  At the end of every month, the mentor and intern should sit down together to review the intern’s progress, loosely based on the guidelines in this Mentor-Intern Evaluation Rubric.  The evaluation process is designed to encourage conversation and professional critique in order to facilitate growth.  Your conversations should be candid, professional, and productive. At the conclusion of your conversation, the mentor should complete this Mentor Evaluation Form.  The form lists categories with standards and then provides an opportunity for the mentor to give feedback on those standards. The student will later have access to the mentor's scores and comments and will be able to comment on, but not change, their evaluation. Leadership Standards:
An excellent student leader will: 
- think critically and creatively about their project 
- actively engage with their mentor, other students, and community members and present their results to the public when applicable
- take the initative to drive their own project forward and seek assistance from their mentor when needed,  including finding project-related work to do on their own while waiting for mentor feedback.

1 = fails to meet all standards
2 = needs improvement in most standards
3 = meets most standards, needs some improvement

- take initiative for executing their project, with their mentor's help and guidance
- show up on time (even a few minutes early) for meetings and meet deadlines
- communicate promptly and effectively, including checking school email daily
- take notes during meetings with mentors and others, set and record work deadlines
- manage their time so that quality work is completed on time
