Housing Placement Form

Please use this form to record a permanent housing placement of any homeless person served in your community. To avoid duplication, for placements into PSH (including HUD VASH), the provider (and not the client's Navigator) should complete this form upon move-in. For all other placements, the Client Navigator or other staff working directly with the client may complete this form. To help capture all housing placement and determine the percentage of all housing placements going through the Coordinated Assessment and Housing Placement (CAHP) System, please complete this form even for clients who were not matched through this system.
