I consent to my child participating in County Badminton activity including training and matches.  I understand that participation in this activity requires compliance with Badminton England (BE) and Avon Junior County Badminton (AJCB formerly ASBA) and Avon Badminton Association (ABA) safeguarding policies, disciplinary codes, and other relevant policies which require respect towards coaches, volunteers, team mates and opponents at all times.  I will support my child in abiding by these commitments and help them have fun and enjoy the sport of Badminton.  

I understand that participation in these activities requires physical exertion and may carry risk of injury or illness.  I accept these risks on behalf of my child.  I have noted any relevant medical information about my child on this consent form, and if needed supplied an attachment with further information.  I understand that Avon Badminton and the coaches who work with Avon Badminton can only be considered aware of medical information included on this form.  Should important information about my child change, such as medical conditions or emergency contact details, I undertake to update Avon Badminton accordingly by requesting and submitting a new copy of this form.  I shall also ensure my child brings any necessary medication to sessions.

I consent to the data on this form being stored by Avon Badminton and agree they may use it for the purposes of badminton and contacting me regarding information relating to badminton.  I understand they will not share my data with any commercial third party without my consent.  I understand and agree that some or all of this data may be shared with Badminton England (BE) and/or entered onto the BE membership or other administrative systems that Avon Badminton may use from time to time.  I agree that this data may be shared with other parents or volunteers for the purposes of badminton related activities.

I give my permission to use photographs of my child participating in badminton for promotional and news purposes and in media publications (eg ABA website, social media, newspapers).  I agree that Avon Badminton coaches may occasionally film badminton activity my child is participating in for the purposes of providing my child coaching, tactical analysis or similar input.  I understand such images will deleted at the end of each session and not retained.

I understand there may be sme events that require my child’s travel to away venues in the charge of an AJCB or ABA representative.  I understand I shall be made aware of any travel requirements or events in advance.  Under these circumstances I give my authorisation for representatives of AJCB or ABA to take charge of my child in my absence at away venues.  

I understand the information I have provided on this form will be retained by Avon Badminton and assumed by Avon Badminton to remain current in perpetuity, unless or until amended by me, or until my child ceases to participate in of Avon Junior County Badminton.  In the event I wish to update my child’s data I shall obtain and submit a new registration form to Avon Badminton. In the event I wish to amend any of the above permissions I shall advise Avon Badminton accordingly by emailing the Avon Chairperson and advising them to that effect.
