School Security Infrastructure Software and Technology Attestation Form
Iowa Homeland Security and Emergency Management is responsible for developing a list of approved organizations to provide school security infrastructure software or technology to Iowa schools. Organizations wishing to be on the approved list must attest that they meet the statutory requirements listed below. This form must be filled out in its entirety for potential placement on the approved organization list.

Is your software and/or technology currently designated by the secretary of homeland security as qualified anti-terrorism technology under 6 U.S.C. §441 et seq.? If yes, please send supporting documentation to CIP@iowa.gov.  *
The organization agrees that any false statement or misrepresentation regarding meeting the statutory requirements of school infrastructure software or technology related to school security will result in denial of placement on the approved organizations list. Furthermore, the organization agrees to the following definitions as they relate to the requirements above.

1. "Associated" means connected with, or a working relationship with.
2. "Open-source data" means data that is freely accessible to anyone, meaning it can be used, modified, and shared without restrictions.
3. "Third-party data" means data collected by a third party or entity which lacks a direct relationship with the user for which the data relates.

Additionally, by submitting below, the organization further agrees that it will, on a quarterly basis, submit a new School Security Infrastructure Software and Technology Attestation form to Iowa Homeland Security and Emergency Management attesting that its unholstered firearm detection software or technology continues to meet the requirements provided in the attestation section above. 

The organization further acknowledges and agrees that if it fails to timely submit this attestation, or if it is unable to attest to all matters provided in the attestation form, Iowa Homeland Security and Emergency Management will remove the organization from the list of approved organizations as provided in Iowa Code section 423F.3(6)“a”(5)(a)(viii).

The organization acknowledges that Iowa Homeland Security and Emergency Management reserves the right to investigate any organization's attestation in order to validate their responses, and that self-certification through this web-based platform may - but does not necessarily guarantee - the organization's inclusion on the list of approved vendors.  *
