Dear Parent / Guardian of 6th grade student:

During this school year, your child is required to have a physical examination for school. You may choose to have your child receive a private exam with their physician or they may receive one in school with the school physician. Please indicate your preference below. School physicals will be conducted during the school day in the nursing office.  Please let me know if you would like to be present during your child’s examination.
