Homework Hangout Consent Form
This permission form is providing permission for your child to be signed up to participate in our school's Peer Tutoring/Homework Hangout program as a tutor or tutee. The program runs after school hours.
We ask parents and guardians to please submit this consent form to confirm they understand how the program functions and permits the student to participate.

This initiative is strictly run within Monarch Park Collegiate.
Due to the current COVID-19 protocols, the program will run online through Zoom, and in person with appropriate protocols in place.
All participants are to follow our strict rules and regulations; if anyone fails to do so, our team and supervisor will take action to address the issue.

Zoom is a web-based video conferencing tool that allows users to meet online, with or without video. Participants must adhere to all TDSB policies including the Human Rights Policy and Equity Policy.
I give consent for my child to use this app for the purpose of participating in the Peer tutoring program.
It is expected that participants in the Peer Tutoring Program: DO NOT share the Zoom class session links with anyone who is not registered; DO NOT take screenshots or recordings of the other participants; BE kind, patient and respectful of the other participants; REFRAIN FROM the use of any profanity – written, drawn, displayed or spoken; BE MINDFUL of what is shared *****Bullying, harassment or inappropriate behavior will result in immediate removal from the sessions.
Student and Parent/Guardian agree to and understand the outlined expectations.
Peer Tutoring/Homework Hangout will take place on a weekly basis. An MPC staff member will be present at all in person sessions, however, will ONLY be in the main meeting space virtually, while students will be in breakout rooms, unsupervised.
