USA Triathlon Quality Control Complaint Submission Form
USA Triathlon strives to ensure that all sanctioned events meet and maintain the standards set forth in the sanctioning agreement signed by all Race Directors when they sanction with USA Triathlon. This program is intended to provide a process by which USA Triathlon can ensure those standards are met and provide disciplinary guidelines for when they are not.
This form is intended to be a way for athletes, parents, spectators, or any other concerned party, to confidentially alert USA Triathlon to any unsafe, unethical or concerning behavior they have personally witnessed at a USA Triathlon sanctioned event. Please note this is not meant to handle any disputes over athlete cheating, timing disputes or other disagreements in event formatting or . Additionally, this form is not intended to receive SafeSport complaints, those should be directed to your local law enforcement and the USA Triathlon SafeSport Coordinator.

Size of the Swim Wave (Waves must be under 150 unless previously approved by USAT)
Water Quality (Must meet guidelines specified by testing entity)
Aid Stations (1 per every 1.5 miles on the run course)
Secure Transitions (Transition areas must be secured by some sort of fencing or other containment)
Failure to Follow Stated Refund Policy (Note: refund policies are at the discretion of the race director and "No Refund" policies are allowed)
Insufficient Lifeguards (1:35 in ocean and 1:50 in all other bodies of water)
Unsafe High Traffic Intersection (These intersections must be staffed by Police and not volunteers)
Bicycle Bar End Plugs (must be available from race director)
Cancellation Without Cause (race director must give sufficient warning and reasoning for cancelling a race)
