I give my permission for the SSC to use my image(photo or video) to promote club events and activities on the SSC website, orother SSC social media sites:
*
I understand that: 1. By submitting this application form, the applicant agrees that the SSC and/or its coaches will not be responsible for accidents or losses – however caused – and agrees to release the club and coaches from any damages, or claims which may arise as a result of such accident or loss. 2. The city has the right to cancel our ice time due to tournaments or special events. In the event that the city cancels a session, it will be made up but sometimes unavoidably it cannot be done at the regular time or day for that session.
