As parent or legal guardian of the above named child, I hereby grant permission for him/her to participate in the Perryton Prime Time Track Events. I acknowledge the fact that he/she is physically able to participate. I acknowledge that any event or activity can be dangerous, and that participation could lead to bodily injury or death. In consideration of participation in Perryton Prime Time, I HEREBY ASSUME THE RISK OF THE ABOVE NAMED CHILD PARTICIPATING and accept full responsibility to pay expenses for medical care that may arise from said participation. I hereby release Perryton Prime Time, and Coaches from all claims that may be sustained by my child. I authorize the Perryton Prime Time, Coaches and staff or designee to select hospital facilities and/or physician of choice and authorize treatment of the above-named athlete on an emergency basis in the event such treatment becomes necessary while participating in Perryton Prime Time trainings, practices, and all other events. 

I also give permission to use his/her photographs in Perryton Prime Time media.
