This questionnaire is for anyone over 18 who identifies as "in recovery" from addictions, mental ill health or family dysfunction. It is not associated with any group or recovery programs.

This is 100% confidential and we do not intend on collecting identifying information, simply your experience. All answers will be kept and stored according to Google Terms of Service.

By completing this questionairre you consent to your information being used directly or indirectly to inform a person centred understanding of recovery in future publications or presentations associated with An Authentic Journey book series and related authors.
