Palena Netball Club 2022 JUNIOR selections
This form is to express interest in attending JUNIOR selections and playing for Palena in 2022.

Details surrounding COVID restrictions, arrival details, deposit payments, uniform sales etc. will be posted closer to the date.
SATURDAY 30th OCTOBER 2021
11 & under (year of birth 2011 & 2012) and
13 & under (year of birth 2009 & 2010)
8:30am-10.00am - arrive 8:15am for sign in and to start warm up
15 & under (year of birth 2007 & 2008)
10.15am-12:30pm - arrive 10.00am for sign in and to start warm up
17 & under (year of birth 2005 & 2006)
12.45pm-2:30pm - arrive 12:30pm for sign in and to start warm up

If player has own email. Team placement will be sent to both emails if provided.
Player's 2021 team (list club played for if not Palena, list year if not 2021, answer "None" for first year club player)
Examples: 15c, 13e Yellow, 11b 2019, ClubXYZ 17b, None
Acknowledge deposit requirements
A $50 deposit is required to be paid at or before selections. This deposit will only be refunded if the player is not placed in a team - not for change of mind or circumstance. Players will no be placed in a team without a paid deposit.
To secure team placement, I acknowledge that I will be required to pay a non-refundable deposit of $50 on or before selections day
Please nominate the position(s) the player will be trialing for - First Preference
Please nominate the position(s) the player will be trialing for - Second Preference (optional)
Please nominate the position(s) the player will be trialing for - Third Preference (optional)
Attendance is required. Team numbers may be capped this year and as such, by not attending the player acknowledges that this may exclude them from being placed in a team.
The player will attend selections
The player is unable to attend selections but the club will be emailed/contacted prior to October 23rd to request consideration for team placement
