Non-CMC Guests: Canvas Access Request

This form is for current CMC faculty who are teaching a Canvas course and wish to add someone to it who is not affiliated in any way with Colorado Mountain College, such as a member of an accreditation agency or a guest lecturer.
(Please do not ask your guests to submit this form; we need to know who is sponsoring whom as a guest in Canvas, thanks.)

This form is only a preliminary step: you will also need to submit the paperwork provided below.
This paperwork is also posted online in Basecamp at and should be submitted to the Office of Innovations in Teaching & Learning:

There are 3 pieces of paperwork:
(1) the policy governing guests in Canvas;
(2) the CMC Canvas Guest Request form which needs to be filled out by you and signed and approved by your dean or instructional supervisor;
(3) Guest Agreement form to be completed and signed by your guest.

Please allow 2 weeks after we receive your paperwork for your guest’s access to be created.
