UDSD Communications & Media Services Videography & Photography Request Form 2026-27

We strive to provide quality videography and photography services to schools and departments across the District. While we would love to be able to provide services for every event and activity, our ability to meet each need is not infinite. We are asking that you access media services judiciously in line with our capacity at this time. Please accept our apologies in advance for any denied requests.

Please complete and submit this form to request media services for your school or departments' upcoming event/activity. A form must be completed for EACH event requiring media services and/or video recording support.

All requests must be submitted at least two (2) weeks prior to  your event/activity. You will receive an automated email indicating that your request has been received. Please contact Jason Taylor at jtaylor@upperdarbysd.org if there are circumstances that prevent you from meeting the 2-week timeline.

IMPORTANT - Your request is PENDING APPROVAL based on scheduling availability. Your event is not approved until you receive a confirmation communication from Jason Taylor.

IMPORTANT - If anything changes  (date, time of event, location, etc). you must email Jason Taylor to inform him ASAP. Please understand that your adjusted request will need to be re-evaluated for approval.
