The purpose of this Census Form is to collect some key statistical data on the Xiong Clan. The data will solely be used for statistical purposes in furtherance of and consistent with XUN goals and missions.

Please fill out the form completely and return the completed form to your local City or State Representative. Only fill out one form per household. Household includes all family members, related by marriage (either by cultural custom or legal), adoption, or birth, residing in the same house.

Please provide information for the rest of the household members.  For each educational level, provide only the number of people who have achieved only up to that level.  For example, if mom, daughter, and son all achieved high school diploma but only mom went on to achieve a master's degree, then you would record that there are two people with high school diploma and one person with a master's degree.
