2025-2026 Middle School Club Registration Form
Complete the following registration form. Acceptance into these clubs is based on availability, therefore it is important to select your top 3 choices. 

All club invitations will be sent out to accepted club applicants between September 22nd - 26th. Invitations will include club dates, late bus information, and policies. If you do not receive a club invitation, it means you were not selected for that specific club.
