I understand that it's my responsibility to organise transport, accommodation and supervision for my child:
If you're able to help out with transporting other riders, please indicate how many seats and bikes you're able to take (including your child):
Would you like to be the Team Manager at this event? This role will require you to collect any team packs, hang on to the first aid kit (which CMS will supply) and ensure the riders are at the start line on time. The rest of the supervision is the full responsibility of the child's family.
Yes I would love to!
If your child is in Year 8, 9 or 10 and has any new medical information not currently listed on his/her Health Form, please advise:
I give permission for my child to travel in a private vehicle, with an approved driver, to 440 MTB Park in Auckland for the North Island MTB Championships from Saturday 21 March to Tuesday 23 March 2021. All our approved drivers hold a valid full driver's license and a vehicle WOF.
I will ensure that my child wears the correct Cambridge Middle Schools sports uniform (to be provided) at the event at all times. Non-CMS branded clothing will not be permitted at the event.
I understand that the payment needs to be made prior to my child being entered into any of these events.
