Time: Arrive at Golf Club at 8.15 am and tee-off from 8.30

Venue: Palmerston North Golf Club, Brightwater Terrace, Palmerston North

Finish time: Approx 2 pm. Please fetch your son from the golf club.

Dress: Normal golf dress with collared shirts. Remember caps and sunscreen or rain gear.

Cost: $15 payable by 6 May

Open to all golfers - need to supply own equipment.

 I approve of my son, details below, participating in the  PNBHS Golf Champs being held at Palmerston North Golf Club on Wednesday 8 May 2024. 

 I understand that: 

·        My son needs to travel to and from PNGC privately.  

·        I am required to pay $15 by 5.00 pm by 6 May 2024 for my son to participate in this activity and that I need to contact the finance office if I am unable to meet this deadline. 

.        My son needs to supply his own equipment.

·        My son will be subject to the Fundamental and Basic school rules during the entire trip. 

·        My son should take part in such activities and such necessary duties as may be required by Mr Smith or Dr Smith. 

·        I need to notify the Main Office prior to departure of any changes in the medical conditions of my son. 

·        I authorise the obtaining of medical assistance, if in the opinion of Mr Smith or Dr Smith, such action is necessary. 

·        I need to notify Mr Smith prior to departure of any specific circumstances that will require his consideration during this activity.
