Noteables Student Information & Parent/Guardian Permission Form 2024-2025
Parents/Guardians:
Please read the following Parent/Guardian Permission Statement and Code of Conduct for the Noteables 2024-2025 Season and complete the following information as completely and accurately as possible before pressing "submit" at the bottom of the page.
I understand that my child will participate in The Noteables Show Choir for the 2024-2025 school year. I hereby grant permission for my child to attend all events in which the choirs will participate, including but not limited to out-of-town events. I understand that my child will be transported to these events by a school-approved vehicle, i.e. school buses, charter buses, or private transportation.
I also understand that each trip will be chaperoned by parents or directors. The choir will carry a first aid kit, and chaperones may give minor first aid treatment. In the event of an accident, I grant chaperones and director the authority to give a physician permission to administer medical aid to my child. Any special health needs and/or allergies that the teacher should be aware of must be completed on the student’s health form.
I also agree to represent my Noteables student(s) at least 8 times on Sunday evenings between June 1, 2024 and May 31, 2025 at the Tazewell Bingo Center. If I cannot personally attend, I will find someone who is at least 18 years of age and not a current PCHS student to represent my Noteables student(s) at the Tazewell Bingo Center.
In addition, I agree to adhere by the following Code of Conduct:
-I will encourage good sportsmanship by being a positive role model.
-I will try my best to make show choir a positive experience for everyone involved, i.e. participants, directors, other parents, and spectators.
-I will insist that my show choir member treats other members, directors, parents, and spectators with respect.
-I will do my best to understand and appreciate the rules of the performance place.
-I will show support and appreciation for outstanding performances by any and all show choirs.
-I will be a “TEAM” fan, not a “MY CHILD” fan.
-I will help my child learn that success is measured by the development of skills, not only winning or losing.
-If I have a concern, I will talk to the director, Mrs. Grizzard at the appropriate time and place, i.e. not before, during, or immediately after a performance or rehearsal.
-I will reinforce the school’s drug and alcohol policies and refrain from using alcohol and other drugs before or during performances or at any point on any school-sponsored event.
-I understand that consistent displays or behaviors of poor sportsmanship at rehearsals, meetings, performances, or other events will result in removal from those events; permanent removal will occur, if necessary.
