The member hereby is agreeing to join access to Motivationz Fitness services. Whether it is for a drop in, one time fee, specialty program, or a continuing monthly/yearly renewal, the member acknowledges and accepts the sessions they have agreed to with the package choice or recurring choices they have made with Motivationz Fitness. 

The member shall be entitled to the sessions they purchased and agree to attend within the expiry time period.  

FEES

For all services to be rendered by the trainers/instructors at Motivationz Fitness, the member shall pay the fees for all sessions in full upon execution of this agreement.  

CONSENT

I have been informed, understand and am aware that strength, flexibility and aerobic exercise, including the use of equipment, are potentially hazardous activities.  I also have been informed, understand and am aware that fitness activities involve risk of injury and that the applicant/parent/guardian are voluntarily participating in these activities and using equipment with full knowledge, understanding and appreciations of the dangers involved. Trainers will not be responsible for any accident or loss, however caused, and agree to release executive and coaches from all claims and damage which may arise as a result of, or by reason of such accident or loss. In the case or accident or injury, the applicant/parent/guardian hereby gives permission for the above named child/participant to be taken to a  medical facility for emergency treatment and care may be administered on-site. I understand that Motivationz Fitness cannot be held responsible for injury or accident. 

The applicant/parent/guardian agrees that pictures may be posted on social media platforms. Names will not be released.
