(1) Complete this Google Form and submit it.
(2) Fill out, sign, and turn in the Parent Release and Student Commitment Form (attached later in the application).
(3) Submit a refundable deposit of $50 for the trip cost. Checks can be made out to "SLUH". Anyone who needs financial assistance for the trip should contact Mr. Stephen Deves (email@example.com). We do not want financial barriers to prevent anyone from attending an Immersion Trip!________________________________________________________________________________________________
The deadline for this application is FRIDAY, OCTOBER 12. Since we have a lot of interest and a limited number of spaces, your application must be completed by that deadline. The application is only considered finished once you've completed all three steps above.
