PLEASE REMEMBER THAT DAY CARE TRANSPORTATION IS NOT GUARANTEED AND WE CAN ONLY PROVIDE ONE STOP IN ADDITION TO YOUR HOME FOR THE MORNING AND AFTER SCHOOL.

In order for us to transport your child to day care, we must have a minimum of five students from your school going to that day care location. In addition, day care is limited to the number of seats we have available for transport. If our day care transportation bus to a location becomes full, we will not be able to transport your child until such time as space becomes available.

The deadline for submitting this request is Tuesday, July 8.

PLEASE COMPLETE A SEPARATE FORM FOR EACH CHILD IN YOUR FAMILY. THIS FORM MUST BE SUBMITTED ANNUALLY FOR SHARED PARENTING. YOU MUST ALSO SUBMIT A DISTRICT BUS TRANSPORTATION OPT-IN FORM.
