Please use this form to request a group reservation for the 2022 - 2022 school year. You will need to submit a separate form for each show you would like to attend. For example, if you plan on attending 3 shows in the season, you will submit this form 3 times indicating a different show/date each time.
Please note that submitting this form does not guarantee you will receive the requested performance date. The Education Box Office Manager will contact you within 5 business days to either confirm your reservation or discuss alternate dates.
Once you receive a confirmation email and invoice, your field trip is booked and you must notify the Education Box Office Manager in writing of any requested changes.
Upon receipt of your invoice a 50% non-refundable deposit is required to secure your reservation. The remaining balance must be received at least 21 days prior to the performance date. This date is listed under each production Date. No addition or subtraction of tickets will be allowed following the final payment

Please note our current policy is that masks are optional at the New Hazlett Theatre. Proof of Vaccination or a negative covid test is NOT currently required. Please note these policies are subject to change.
