Dear Student,

We are very sorry that you have had a negative experience of any kind. Please know that we take grievances very seriously.

If you are having a problem, that also means others are having the same problem. We appreciate you allowing us to flag and address potential issues.

This form is anonymous, and we will not provide identifying information to anyone. However, by providing your contact information, it enables us to follow up and validate submissions adequately.

Lastly, you also have the option to request a consultation from The Center for Community Standards and Accountability (CSA). Please note that submitting this form for a referral or consultation does not automatically open a case or notify any involved parties.
