Form Submission Guidelines

By completing this form and pressing the submit button below, I give the Bouvier Health Foundation (BHF), a division of the American Bouvier des Flandres Club, a 501(c)3 non-profit, permission to use the photos I provide in the manner outlined in this submission form. I understand that as long as this permission is granted the photo may or may not be used at the BHFs discretion.

I understand that I may withdraw consent at any time by emailing webadmin@bouvierhealthfoundation.org. For such requests, please allow up to one (1) week for this claim to be processed. You will be notified when it is complete.
